Pressure Ulcer Prevalence

Spaulding Rehabilitation Hospital has a comprehensive wound program to address pressure injuries. The Wound Care Specialist (WCS) is consulted on all patients with pressure injuries. The WCS coordinates the patient's treatment plan, in conjunction with the Attending Physician and other members of the team (nutrition, therapies, infection preventions). We have a Skin and Wound Assessment Team (SWAT) comprised of both nurses and therapists who are consulted for their cross discipline expertise. The SWAT Team provides ongoing education and training to patients, families and staff. This team approach allows for consistent communication and identification of appropriate interventions to facilitate wound healing while actively participating in functional recovery. Staff are also trained through the Wound Treatment Associate program, which helps develop a unit based cadre of nurses to help with wound-related questions and serve as resources to their peers.

Falls
Spaulding Rehabilitation Hospital has a robust Fall Prevention Program. Our proactive Fall Prevention Program mitigates risk from physiological and environmental factors that contribute to an individual’s risk for fall-related injuries. The patient’s risk for falls and fall related injuries and specific ways to prevent falls is communicated to team members and family. Our Fall Prevention video provides interventions to proactively prevent falls. The video has been well received by patients and families. From admission to discharge, our patients and families learn how important falls prevention is to all of us. Our team of advanced clinicians provides an evidence-based falls education program. We perform interdisciplinary post fall safety huddles. The information that is collected from the post fall safety huddle is useful in preventing future patient falls and injuries as the patient’s function progresses through their rehabilitation program.
